The Medication History Pharmacy Tech reports to the Assistant Director of Pharmacy and is responsible for accurately collecting and entering current patient medications into the hospital's information system for utilization by the physicians and providers responsible for admitting and discharging the patient from the hospital. The primary function includes patient interviews and review of internal and external systems with relevant information. The Medication History Pharmacy Tech is responsible for relaying any outstanding transition of care issues to the lead pharmacist and/or physician. The Medication History Pharmacy Tech will also perform limited medication management and/or distribution activities under the supervision of a pharmacist. The Medication History Pharmacy Tech will perform assigned responsibilities according to department policies, procedures, standards and regulations within stated time frames.
1. Meets all standards of the Patient Satisfaction Initiative Criteria in order to make Abington - Jefferson Health (AJH) the hospital of choice for Patients and Employees.
2. Effectively communicates with patients, families, and/or caregivers to perform a patient medication history and construct a pre-admission medication list (PAML).
3. Reconciles and documents the "best possible medication history" (BPMH) accurately and completely in the medical record.
4. Contacts the patient's outside pharmacist or physician to resolve all discrepancies in a timely manner and documents resolution of discrepancies in the medical record so that a physician can review and reconcile the list. Handles patients' own medications to ensure they are secured and documented.
1+ years of Pharmacy Technician experience.
Prior experience with medication history/reconciliation preferred. Retail pharmacy experience preferred.
HS Diploma or equivalent required
Certified Pharmacy Technician (CPhT) required
